Individual Program Plan/Individualized Family Service Plan Meeting Survey

Alta California Regional Center is conducting a survey on Individual Program Plan (IPP)/Individualized Family Service Plan (IFSP) Meetings. These surveys are unique to each client/family based on their IPP/IFSP and a survey link is provided to each client/family individually.

Question asked in the survey include:

• Your name
• Relationship to the client
• Location of the IPP/IFSP
• Your preferred location to hold a meeting
• Your satisfaction for your Service Coordinator
• Satisfaction with the outcome if your newest IPP/IFSP
• Requests for changes on your services and supports
