Planning Services Together
Alta California Regional Center
(ACRC) clients will have a designated Service Coordinator to
facilitate the planning process. The Service Coordinator, along
with the child and their family encompass the Planning Team. They
work together to review options and once the options are agreed
upon, the Service Coordinator puts services and supports in place
to help meet the child’s identified goals. Goals, services and
supports create the Individual Program Plan (IPP), which is
unique to every client. The Planning Team develops services and
supports by building upon the child’s natural supports, and
maximizing the use of generic resources. The Service Coordinator
arranges and helps the family, or caregiver, access referrals to
supports that are identified in the IPP. The Service Coordinator
also initiates Purchases of Service (POS) for Regional Center
funded programs and services identified by the Planning Team.
ACRC supports services that values clients as integrated and
contributing members in their community, while maximizing their
capacity for independence.
